Endovenous Laser Therapy (EVLT)

Endovenous treatments ('from within the vein’) have emerged as effective alternatives to surgery, for the management of varicose veins and chronic venous disease. Rather than surgically removing the problem veins, these techniques involve sealing the veins closed, rerouting blood to nearby healthy veins. These treatments are equally suitable for symptomatic venous disease, or for those bothered by the cosmetic appearance of unsightly varicose veins.
Guided by ultrasound imaging, a catheter is threaded through the vein in the lower leg, and advanced to the top of the target vein, commonly in the groin. Delivery of energy by way of the laser fibre causes direct thermal injury to the vein wall, as the device is withdrawn through the diseased vein. This form of treatment requires tumescent anesthesia, a form of local freezing, along the length of the vein, in order to promote vein collapse and prevent injury to surrounding tissue.
Clinical evidence suggests equivalent, if not superior, effectiveness of endovenous ablation compared to surgical vein stripping, with a reduction in the rate of long-term recurrence.

A proven procedure since the 1930s, varicose and spider veins can be treated by direct injection of a chemical solution or sclerosing agent, resulting in irritation of the vein wall and closure of the vein. Over time, these veins shrink and gradually fade.
Sclerotherapy is an office-based procedure that does not require anesthetic or sedation. If the veins to be treated are not visible outwardly, an ultrasound is used to guide therapy. Several sessions may be required to eradicate all unsightly veins.
Sclerotherapy can be used as both a stand-alone procedure for diseased veins or as an adjunct to endovenous ablation therapies. Speak to a North Toronto Surgical vein specialist to determine which treatment modalities are most appropriate for you.

A single session of sclerotherapy costs $300, a package of three sessions is $750. Most patients will require at least three sessions for symptom improvement.
Surgery

Surgical removal of diseased veins has been performed for over a century. Over the past decade, endovenous ablation has largely replaced traditional open surgical treatment due to a more favourable side effect and complication profile. This technique, however, remains a viable therapeutic option in certain circumstances, mainly depending on anatomy of the problem vein and history of previous vein inflammation.
Surgical vein excision is performed in a hospital setting under general anesthesia. An incision is made at the junction between the diseased surface vein and the underlying deep vein, commonly in the groin. A counterincision is also necessary in the lower leg. The problem vein is then detached and removed (stripped) between these two sites. Removal of individual varicose veins is performed simultaneously, through tiny nicks in the skin.
After vein stripping, pain, bruising and swelling are common. In general, these symptoms last between 3-4 weeks. Return to regular activities can occur approximately 2-4 weeks following the procedure.

The cost of vein ligation and stripping is covered with a valid OHIP card if you meet certain clinical criteria. The current wait time for surgery is 3-6 months.
